（*）法人依法设立的分支机构以自己的名义参与投标时，应提供依法登记的相关证明材料和由法人出具的授权其分支机构在其经营范围内参加政府采购活动并承担全部民事责任的书面授权。法人与其分支机构不得同时参与同一项目的采购活动。
（*）具有中华人民共和国医疗机构执业许可证。
（*）符合《关于对接国际高标准经贸规则推进试点地区政府采购改革的指导意见》（沪财采〔****〕**号）第**条规定的供应商，不得参加本项目的采购活动。
（*）本项目不允许转包。

(c)Specific qualification requirements for this program:(*) This project is aimed at large, medium, small, and micro enterprises, public institutions, other organizations, or natural persons for procurement. (*) When a branch established by a legal entity participates in bidding in its own name, it shall provide relevant certification materials registered in accordance with the law and a written authorization issued by the legal entity authorizing its branch to participate in government procurement activities within its business scope and bear all civil liabilities. (*) Having a practicing license for medical institutions in the People's Republic of China.
